Deep Plane Facelift

Restore volume, definition, and a naturally youthful appearance with the deep plane facelift — performed by board-certified plastic surgeon, Frederick G. Weniger, MD, FACS

What Is a Deep Plane Facelift?

Not every facelift is created equal. The deep plane facelift is widely regarded as the most sophisticated and comprehensive approach to surgical facial rejuvenation — and for good reason. Unlike traditional techniques that address only the surface layer of skin and more conservatively manage the deeper structures of the face, the deep plane method releases complex ligaments that tether the SMAS (Superficial Musculo-Aponeurotic System) in place, allowing the deeper tissues to be lifted more completely and naturally.

The result? A refreshed, naturally youthful appearance that looks like you — just years younger. No telltale tightness. No windswept look. Just balanced, natural rejuvenation that moves with your face.

At Weniger Plastic Surgery in Bluffton, SC, board-certified plastic surgeon, Frederick G. Weniger, MD, FACS combines his surgical expertise with an artistic eye for facial harmony, tailoring every facelift and deep plane facelift to each patient’s unique anatomy and aesthetic goals.

Deep Plane Facelift Before & Afters

Are You a Candidate?

Is a Deep Plane Facelift Right for You?

The deep plane facelift is an excellent option for women and men who are experiencing moderate to significant facial aging and are looking for results that last. You may be a strong candidate if you notice:

  • Deep nasolabial folds or ‘smile lines’ that cannot be corrected with fillers alone
  • Significant descent of the midface, causing a flattened or hollowed appearance
  • Jowling along the jawline that has altered your facial silhouette
  • Laxity in the lower face that makes you look older or more fatigued than you feel
  • Corners of the mouth turning downward and the appearance of frowning
  • A desire for long-lasting results — not just a temporary improvement

The ideal candidate is in good general health, a non-smoker (or willing to quit), and has realistic expectations for their outcome. The best way to know if the deep plane facelift is right for you is through a personal consultation with Dr. Weniger.

The Procedure

Your Deep Plane Facelift Journey

Step One

01

Your Personalized Consultation

Your deep plane facelift journey begins with an in-depth consultation at our Bluffton practice. Dr. Weniger will assess your facial anatomy, discuss your aesthetic concerns in detail, review your medical history, and outline a personalized surgical plan designed to achieve your goals. Dr. Weniger feels strongly about spending time explaining options and the specific reasoning behind the surgical plan- because it is different for every patient. Clarity and understanding is paramount to ensure that the proposed plan aligns with your goal. This is your opportunity to ask every question and understand exactly what to expect.

Step Two

02

The Surgical Procedure

Deep plane facelift surgery is typically performed under general anesthesia and takes approximately 4 to 6 hours, depending on the complexity of the procedure and whether additional treatments are combined. Dr. Weniger makes carefully concealed incisions — typically along the hairline and around the ears — to access the deeper tissue layers. He then releases the key retaining ligaments of the face, lifts and repositions the SMAS and deeper facial structures, removes any excess skin, and closes the incisions with meticulous precision. Because the deep plane technique repositions the foundational structure of the face rather than simply pulling the skin, results appear natural and may last longer and more completely fix drooping than traditional facelift approaches.

Step Three

03

Recovery & Results

Most patients experience swelling and bruising for the first one to two weeks following surgery. You can typically return to walking around the house or even outside the next day ,advancing to walking a few miles a day by the end of the first week!  Other than exercising, activities can be back to normal within two weeks, with a gradual return to full, strenuous activity over four to six weeks. The majority of visible swelling resolves within four to six weeks, though final results continue to refine over several months. Most patients enjoy the transformative, long-lasting benefits of their deep plane facelift for ten years or more.

COMBINING PROCEDURES

Enhance Your Results with Additional Procedures

Many patients choose to combine their deep plane facelift with additional procedures to create a more harmonious, natural, rejuvenated appearance- as opposed to addressing one isolated area of the face.  Dr. Weniger frequently pairs the deep plane facelift with:

Deep Plane Necklift — to address not just laxity, but also platysma muscle banding and extra bulk from deep fat, drooping salivary glands, and bulky deep muscles in order to create the most defined, chiseled jawline.

Blepharoplasty (Eyelid Surgery) — to refresh the upper and/or lower eyelids

Brow Lift — to restore a naturally lifted, open brow position and correct drooping in the temples

Fat Grafting — to restore youthful volume to the midface, temples, lips, or under-eye area.

Laser Resurfacing — to improve deep wrinkles.

Frequently Asked Questions

A traditional facelift primarily tightens the skin and more conservatively tightens the SMAS (fascia) layer. A deep plane facelift goes below the SMAS, releasing the facial retaining ligaments and more powerfully repositioning the underlying fat compartments and muscle layer as a single unit. This produces more comprehensive rejuvenation of the midface, nasolabial folds, and jowls — with results that look natural, not pulled — and longevity that typically surpasses traditional techniques by several years.

Natural-looking results are the hallmark of Dr. Weniger’s approach. Because the deep plane technique works with the face’s own structural anatomy rather than simply pulling surface skin, patients heal with balanced, harmonious results. You should look like a refreshed, rested version of yourself — not like someone who has clearly had surgery.

While no surgical procedure permanently halts the aging process, the deep plane facelift is often recognized as the most durable facelift technique available. Most patients enjoy their results for ten years or more. Maintaining a healthy lifestyle, protecting your skin from sun exposure, and partnering with our LUX Medical Spa team for ongoing skincare can help extend and preserve your outcome.

You can expect swelling and bruising, but very minimal discomfort in the first one to two weeks following surgery. Every patient is different, but patients are often amazed that they have no pain the next day, despite us telling them they probably won’t. Dr. Weniger and his team provide detailed post-operative instructions and remain available throughout your recovery. Dr. Weniger and his staff generally see patients at least twice a week during the first two weeks after surgery.

Most patients feel comfortable returning to social situations within two to three weeks and resume full normal activity, including strenuous exercise, within four to six weeks.

Many of Dr. Weniger’s patients travel not just from Hilton Head Island, Savannah, and Charleston, but also from New York, Arizona, Pennsylvania, Florida, Tennessee, London, and far beyond for the combination of his surgical expertise, personalized care, and the privacy of a boutique practice setting.

Weniger Plastic Surgery also offers Destination & Travel Packages for out-of-area patients, making the experience seamless from consultation through recovery.
